*I understand that the Spring 2022 Event will be facilitated through a third party, Midland57 Media. I allow AdoptOntario staff to share my contact details (full name, work email address and work phone number) with Midland57 Media to schedule and conduct one-on-one presenter technical training and pre-record my presentation/s. I understand that recordings of my voice, video image, name and agency will be used for the duration of the Spring2022 event and will be archived by Midland57 Media and viewable by approved registrants participating through their secure AdoptOntario account, up until Monday, May 30th, 2022. I hereby give my consent to AdoptOntario, to use my image and likeness and/or any pre-recorded presentations of me. A copy of the presenter’s recording will be kept by AdoptOntario for potential future ARE use and recruitment strategies.
